END OF LIFE CARE DISCUSSION
A conversation between a patient, their loved ones, and the healthcare providers about the patients preferences, wishes and goals for care towards the end of life. It involves topics such as code status, treatment options, pain management, comfort measures, life support, hospice care and more.
CRITICAL CARE ADVOCACY
Nurse advocate with critical care experience can provide valuable support including understanding complex medical information, providing emotional support, advocating for the patients needs, coordinating care and facilitating communication during hospitalization.
CARDIAC MONITORING AND LIFESTYLE MODIFICATIONS
RN assistance with common cardiac conditions such as hypertension and congestive heart failure. Assess readings and make recommendations for lifestyle modifications to improve symptoms and work closely with medicinal management, if necessary.
TRANSITIONAL CARE
Supportive care ensuring that the patient's needs and preferences are addressed. Assistance facilitating communication between the patients and healthcare providers, coordinating care and appointments, providing education, and addressing any challenges or barriers that may arise.
HOME SERVICES
Home consultations, assessments or services may be provided in the home setting.
FACILITATE FAMILY DISCUSSION
A nurse patient advocate can play a significant role in facilitating family discussions related to a patients healthcare journey including providing education, creating a safe environment, mediate communication, clarify misunderstandings and support decision making.
As a healthcare power of attorney, a nurse patient advocate can serve as the designated person who has the legal authority to make healthcare decisions on behalf of a patient when they are unable to do so themselves.
Physically or virtually join you or your loved one in doctors’ visits to facilitate questions, voice concerns and interpret on your/their behalf. Privacy will be honored and will only enter conversation when invited. Service includes phone update to designated family member or legal power of attorney.
