Six weeks ago my father-in-law was admitted to the hospital for heart failure. It was the 4th time he had been admitted in a year. No one had explained to us exactly what heart failure was or how to treat it. Our discharge was always rushed and my father in law was always the type to brush things under the rug. After the 4th time, we decided to try a different approach and met Amber. She came over the next day, assessed him, and was able to provide the education we all needed. After that, my father in law was more keen to complying with medications and other restrictions. Amber checked in every day at first to make sure he was doing well. She still is available if we have any questions and to monitor his progress. We are so thankful for her and we feel confident we wont be going back to the hospital any time soon! K.G.

My husband and I hired "Your Nurse Champions" after a long battle in the hospital lasting over a month. My husband was admitted for a simple pneumonia that led to long term ventilation and many complications. I felt completely lost and anxious every day trying to figure out what was happening. My mother moved to Florida about nineteen years ago to escape from New York. She was independent at the time, but as the years went by she needed more and more help. Her friends in the community could only do so much. There came a point we were making frequent trips down to check on her, and we were getting worried about her safety. I did a google search and found Amber listed as a nurse patient advocate in the area. I called and she called me right back. I was so happy to find a nurse doing something like this. She knew the area and was close to mom. It was eventually determined she would need to be placed into an assisted living facility. Amber not only checked on mom frequently, but she was also able to be at the hospital when she was diagnosed with a UTI and got confused at home. I don't know what we would have done without her! Her expertise, excellent communication and assessment skills were crucial. She continues to check on mom and will work with her when she needs more help down the road. I have been suffering from high blood pressure, being 30lb overweight, low energy, and no consistent motivation for the last 3 years. When amber first told me about her program, I was skeptical.
